Telemedicine Informed Consent

1. Nature of Telemedicine
Telemedicine involves the use of electronic communications to provide clinical services. This may include video calls, audio calls, secure messaging, and electronic prescribing.
2. Potential Benefits
- Convenient access to licensed providers from your home or workplace.
- Continuity of care across state lines where providers are licensed.
- Timely access to prescription therapies, including compounded medications.
3. Potential Risks
- Technical failures such as dropped calls or poor video quality.
- Limitations in the provider’s ability to examine certain physical signs.
- In rare cases, incomplete or delayed diagnosis compared to in-person care.
4. Patient Responsibilities
- Provide accurate health history and disclose all medications and conditions.
- Follow treatment and follow-up recommendations.
- Arrange in-person care when advised or when necessary.
5. Alternatives
You may choose in-person care with another provider at any time instead of telemedicine.
6. Confidentiality
Your information will be protected in compliance with HIPAA and state laws. Electronic communications are encrypted, but no system is completely risk-free.
7. Financial Responsibility
You are responsible for all charges associated with telemedicine services, including membership fees, labs, and medications.
8. Consent
By using DW Health First telemedicine services, you acknowledge that you have read and understood this consent, and you voluntarily agree to participate in telemedicine-based care.
9. Additional Treatment-Specific Consents
In addition to this general consent, treatment-specific informed consents (such as for GLP-1 weight loss therapy or peptide therapy) are provided to patients inside our secure patient portal (Practice Better) prior to starting treatment. Patients must review and sign these consents electronically before therapy begins.
